General Medicine Module

Chronic condition management, SOAP notes, medical certificates (7 types), AI disease coach, and NICE/WHO aligned protocols.

Chronic condition management

The General Medicine module tracks chronic conditions with longitudinal clinical markers.


Adding a chronic condition:

  1. Patient Profile > History > + Add Condition.
  2. Select the ICD-10 code (searchable) and condition name.
  3. Set status: Active / Controlled / In Remission / Resolved.
  4. Enter clinical markers based on condition type:

Diabetes Mellitus:

HbA1c (%), fasting glucose (mmol/L), 2-hour post-prandial glucose, latest eGFR, last eye exam date, last foot exam date


Hypertension:

Systolic and diastolic BP trend (pulled from vitals), SCORE2 cardiovascular risk score, current medication class (RAAS, CCB, thiazide), LVH status


Asthma/COPD:

GINA step (1–5), spirometry (FEV1, FVC, FEV1/FVC ratio), exacerbation log, rescue inhaler use frequency


CKD:

eGFR trend, proteinuria, KDIGO stage, dialysis status


Heart Failure:

NYHA class, LVEF (%), GDMT tracker (see Cardiology module)


All markers are plotted as trend charts. The AI Chronic Disease Coach button appears at the top of each condition card and generates a management plan aligned to NICE/WHO 2024 guidelines.

Medical certificates (7 types)

Clinit generates 7 ready-to-print medical certificate types:


  1. SICK LEAVE CERTIFICATE

Auto-numbered: SL-YYYY-NNNN

Fields: patient name, national ID, diagnosis (ICD-10 or free text), rest days recommended, employer name (optional), return-to-work date

Restriction list: optional field for duties to be avoided


  1. FITNESS FOR WORK

"Examined and found FIT / UNFIT to perform duties"

Includes: type of work, physical restrictions, validity date


  1. FITNESS FOR SPORT / PHYSICAL ACTIVITY

Resting HR, BP, BMI, cardiovascular assessment findings

Graded: General exercise / Competitive sport / Professional sport

Validity: 12 months from date of issue


  1. FITNESS FOR DRIVING

Standard Egyptian RTA format

Visual acuity (corrected and uncorrected), colour vision result, hearing screen, neurological exam findings

Validity: 1 year (under 60), 6 months (60+)


  1. SCHOOL / UNIVERSITY ABSENCE

Diagnosis, dates absent, educational institution field

Return-to-school recommendation and clearance date


  1. INSURANCE MEDICAL REPORT

Comprehensive: PMH, current medications, examination findings, investigations attached

Formatted to standard Egyptian insurance company requirements


  1. TRAVEL FITNESS CERTIFICATE

"Fit to travel" by mode (air / sea / land / all)

Special conditions: oxygen required, stretcher, medical escort needed

Validity date (for airline submission)


Workflow: Patient > Session > Documents > [Select type] > Review pre-filled fields > Print or Send PDF via WhatsApp.

All certificates are auto-numbered, signed with clinic stamp in the template, and stored in the patient Documents tab.
