Eliminating Paediatric Dosing Errors with Weight-Based Calculators

Dosing errors in children are 3× more common than in adults. Clinit's weight-based dosing calculator pulls the child's latest weight automatically and computes mg/kg doses at the point of prescribing.

The Scope of the Problem

Paediatric medication errors are disproportionately common and disproportionately serious. A 2023 WHO report found that dosing errors account for 40% of all medication errors in children, and that manual calculation is the primary contributor.

How Clinit Addresses This

When a clinician opens the prescription tab for a paediatric patient, the system:
  • Pulls the most recent recorded weight (from vitals or the growth module)
  • For each drug added, prompts the clinician to confirm the dose route (mg/kg, mg/kg/day, etc.)
  • Computes the absolute dose and flags if it exceeds the published maximum single dose or maximum daily dose

Drug Library

Clinit's paediatric drug library includes over 400 commonly prescribed medications with:
  • Standard dose ranges per age band and weight band
  • Maximum doses (single and daily)
  • Renal and hepatic adjustment flags
  • Common formulations (suspension concentration, tablet strength)

Bilirubin Nomogram

The Bhutani nomogram for neonatal hyperbilirubinaemia is embedded in the newborn section. Enter the bilirubin level and postnatal age in hours, and the system plots the result on the nomogram, categorises the risk zone, and suggests the appropriate management pathway.

The Bottom Line

Clinit removes the arithmetic from paediatric prescribing. Clinicians confirm the reasoning; the system does the calculation and the safety check.
