6. Insurance Medical Report
- Comprehensive: past medical history, current medications, examination findings, investigations
- Formatted to standard insurance company requirements
- Attachments: lab results, ECG, imaging reports
7. Travel Fitness Certificate
- "Fit to travel" by specified mode (air, sea, land)
- Special conditions field (oxygen requirement, stretcher, escort)
