Why Generic EMR Templates Fail Specialists

A cardiologist documenting a stress-test result has very different data needs than a dentist charting a caries lesion or an optometrist recording refraction values. Forcing both into the same free-text fields wastes time and produces records that are hard to audit or analyse later.

Dental: The Tooth Chart

Each patient visit in the dental module opens with an interactive odontogram. The doctor taps a tooth, selects a finding (caries, restoration, extraction, crown, implant), and the system updates both the visual chart and the structured data layer simultaneously. All findings are timestamped and linked to the visit, creating a longitudinal record without manual charting.

Drug prescription templates prepopulate common post-extraction regimens (amoxicillin, metronidazole, ibuprofen), which the doctor can modify or approve with a single tap.

Optical: Refraction & Visual Acuity

The optical note includes grids for right and left eye: sphere, cylinder, axis, near/distance VA, and intraocular pressure. A lens Rx card is auto-generated from the refraction data and can be printed or sent to the patient as a PDF β€” no re-entry required.

Cardiology: ECG Tags & Risk Scores

Lesion descriptors (size, colour, border, surface texture) are structured fields that feed into a searchable database β€” useful for tracking lesion progression over time.

Pediatrics: Growth Charts & Vaccination Tracker

Pediatric notes automatically plot height, weight, and head circumference on WHO growth-for-age percentile charts. The vaccination tracker cross-references the entered vaccines against the national schedule and flags any overdue doses.

Physiotherapy: SOAP & Exercise Library

Physiotherapy notes follow the SOAP (Subjective, Objective, Assessment, Plan) structure with fields calibrated to musculoskeletal and neurological presentations. The Plan section includes an exercise library from which the doctor assigns home exercises; the patient receives a printable or digital PDF of their programme.

Custom Fields

Beyond the built-in specialty templates, Clinit supports clinic-defined custom fields β€” free-text, number, date, dropdown, or checkbox β€” added to any section of any template. Changes propagate immediately to all new visits.
